What to Expect

As a patient at Renaissance Physical Therapy you can expect us to be warm, friendly, respectful and of service to you.  You can count on us being committed to your care and to you achieving your rehabilitation goals.

Your first visit consists of a thorough, one-on-one evaluation with one of our experienced therapists, the outcome of which is an individualized care plan to best address your condition. We’ll also begin educating you on the nature of your condition and underlying factors contributing to it, and will instruct you in exercises so you can begin to manage your condition at home. 

Your therapy will also include hands-on, manual therapy techniques to help reduce pain and increase your function.  If needed, treatments such as ultrasound and electrical stimulation may be used to help with pain as well.  Additionally, education is always a key component of your program throughout your course of care.

Every visit to our clinic is, in part, an ongoing re-evaluation of your program to ensure we’re staying on target to achieve your goals.  We simply request that you always provide us with feedback along the way.  If we’re doing well for you, let us know about it…and let your referring provider know about it!  If there’s something we can do better, please let us know that too.

Insurance and Billing

As a courtesy to you, Renaissance Physical Therapy will verify your physical therapy insurance benefits and bill your insurance carrier for our services.  We generally verify your coverage within one to two days of your first appointment but will gladly pre-verify your coverage before your first visit as well.

Please note that while we verify your benefits for you, it is ultimately best for every patient to know his or her insurance benefits before beginning care with us. We will even provide you with the list of questions to ask your insurance carrier so you get a thorough and accurate benefit quote.

Listed below are links to our unique sets of New Patient Paperwork.  Please check with our office to see which set of new patient paperwork you will need to complete. 206-361-2225206-361-2225.  Once we’ve communicated to you which set of paperwork to complete, please select the appropriate link below to obtain the correct paperwork packet.

  1. Regence Paperwork  - Please fill out this packet if you are using Regence as your insurance carrier.
  2. Premera Paperwork - Please fill out this paperwork if you are using Premera, Lifewise or BCBS insurance.
  3. Auto Accident Paperwork - Please fill out this paperwork if you are using an auto insurance carrier.
  4. New Patient Paperwork  - Please fill out this paperwork if you are using any other insurance carrier.

Physical Therapy Prescriptions

Washington is a “Direct Access” state meaning that a patient can legally access physical therapy services without a medical professional’s referral or prescription.  However, many insurance carriers require a physical therapy prescription to pay for therapy services.  For this reason, we strongly recommend you verify with your insurance company whether or not you need a prescription before scheduling with us.  If you’d like, we will gladly look into this for you before your first visit.

If you will be seeing your medical provider soon and want to obtain a physical therapy prescription from him or her, you can download one of our prescriptions here, have your provider sign and date it, and bring it to your first appointment with us.

Driving Directions

We are just off Interstate 5, off of exit 177 (Lake Forest Park/Edmonds ferry exit). Our office is located in the Ballinger Village Shopping Plaza near Thriftway, 24Hour Fitness and Rite Aid.

We are easily accessed from Shoreline, Lake Forest Park, Mountlake Terrace, Edmonds, North Seattle, Kenmore and south Lynnwood.

First Visit

  • Wear loose, comfortable clothing and appropriate footwear
  • Arrive a few minutes early to complete and review new patient registration forms
  • Meet physical therapist for thorough evaluation of injury or condition
  • Review goals and specific needs to develop an individualized treatment plan
  • Determine the frequency and duration of your course of physical therapy based on rehabilitation objectives
  • Discuss patient expectations for maximizing therapy visits and home exercises. Active participation from the patient is vital to a successful recovery and rehabilitation experience
